CommunityCare is covering the cost of the service – including doctors, health coaches, the latest and greatest tech sensors and more. Reduce medications, improve how you feel and reverse type 2 diabetes with Twin Health!

Twin Health is a covered no-cost benefit for members ages 18 and older with type 2 diabetes who are enrolled in a CommunityCare medical plan. Members who are eligible will be invited by Twin Health to join the program.
