  • Copy of your Oklahoma Health/Life Insurance License(s)
  • Copy of your Errors and Omissions Declaration Page
  • Agency/Producer Appointment Application
  • Agency/Agent Agreement (Medicare)
  • HIPAA Business Associate Agreement (Medicare)
  • W9 Form
  • Authorization Agreement for Direct Deposit Payments
  • Copy of your AHIP Training Certificate (Medicare)
  • Copy of your FFM Training Certificate (Individual)

Complete your AHIP certificate with CommunityCare at a discounted rate! Agents who complete their AHIP certification through the CommunityCare link will no longer be required to send in a physical copy of their AHIP certificate.
