CommunityCare invites you to take a Personal Health Risk Assessment from MediKeeper.

This online assessment will help give you a better idea of your overall physical, mental and emotional health.

This assessment takes about 20 minutes to complete. Use the link below to get started.

Your username will be your member ID number (without the dash). You will need to register with an email and create a password before you can get started.

about accessing the Health Risk Assessment provided by MediKeeper

You may register and complete the HRA online before your policy goes into effect.

The top panels of this page have instructions for registration. On the MediKeeper login page you can register for the first time. At the bottom of the page under, Don’t have an account?, click REGISTER NOW. You will then be asked to fill out the form so you may complete the HRA online.

Make sure you are at the login page for MediKeeper and not CommunityCare’s Member Portal. The first time you will need to register by clicking REGISTER NOW at the bottom of the page.

Your username is your CommunityCare Member ID# listed on the front of your CommunityCare Member card, 11 digits without the dash.

Click on “Forgot Password” on the login screen and follow the prompts.

If your password is still not working, we can help. Call us at 918-594-4031 for a temporary password.

No, an email address is not required to register. However, it is recommended to have an email address so you can set up notifications and fully use all of the features.
