CommunityCare Life and Health Insurance Company utilizes the CommunityCare PPO Network within Oklahoma. To find a participating provider, Click Here to return to the CommunityCare Life & Health PPO page and select the appropriate provider directory network.

To find participating physicians and facilities outside of Oklahoma:

CommunityCare Life and Health Insurance Company provides an in-network level of benefits for services delivered outside of Oklahoma through a national PPO network, Valenz Health. To find a participating provider outside of Oklahoma, follow the steps listed below.

1. Click Here to go to the Valenz Health Provider Search.
2. Select Name, Specialty, Facility, or Services from the drop-down selection.
3. Enter the information for your search in the search bar, then click Search Nearby to access the search results.
4. To refine your search and select another location click on Refine Search on the search results page.
5. Click on the down arrow next to My Location and un-mark the box next to my location. Enter the zip code and any other location information that is available. Scroll down and click on Apply to view the refined search results.

Please note: This network is available only outside of Oklahoma.
