CancerCARE Program

CommunityCare is pleased to offer CancerCARE, a product and service of INTERLINK® COE Networks & Programs. The CancerCARE program supports cancer patients by offering access to experienced oncology nurses and physicians and ensures that CommunityCare members receive the most appropriate, best outcome cancer care available.

CancerCARE nurses can help form questions to ask doctors about treatments, explain diagnosis and treatment options in understandable terms, and help locate resources that will be beneficial throughout the treatment process. CancerCARE communicates with members and doctors as care is being planned to ensure our members receive the highest quality treatment and best possible outcome. These services are included with several health insurance plans and are available to our members at no cost.

How are CommunityCare members referred to CancerCARE?

CommunityCare members with a cancer diagnosis may be referred to CancerCARE by filling out the referral form on the CancerCARE website at https://cancercareprogram.com/ccokregistration/ or by calling 1.877.640.9610 . CancerCARE prefers to learn of patients as close to diagnosis as possible.

Once registered in the program, you will be assigned to an oncology nurse expert who will act as your advocate and ensure you receive the right treatment, at the right time, at the right place.
